Thats the giveaway ... This uses the Centos 5.5 IB support.  I don't 
know if the drivers in it were updated for your card.  That ib_uverbs is 
loaded, but it doesn't support verbs suggests that the driver may not be 
fully functional.

The IB in Centos 5.x is somewhere between 1.3 and 1.4 OFED.  If you can 
get the openfabrics stack on there, say 1.5.1, you should be fine (1.5.2 
has released, but we haven't played much with it yet, apart from some 
Qlogic cards, which didn't like it much).
