I can reproduce the problem with a (2+1) configuration.  It will fail when the
client is on a separate server and when the client is on one of the brick
servers.  It also appears that it is slightly easier to reproduce when the
client is on a separate server.
On a (2+1) I could reproduce failure with as few as 4 readers when the client
is on a separate server.  When the client is on a brick server, I can reproduce
with 5 readers.
For bigger volumes (more servers/bricks) I have seen failures with as few as 3
readers.
General trend: More bricks/servers cause more failures.  More readers cause
more failures.

Smells like a race.
