This is odd. Not sure why it is still finding a hole in the layout of /.

This is fine. The application is probably cd'd in a directory and created a
subdir. Because of the missing parent dir the mkdir of the subdir failed on
the newly added server. Eventually when lookups arrive from the ancestors
(or during rebalance) the missing dirs will be created.
